The Mediterranean diet is not a strictly defined diet, but rather a traditional dietary pattern observed over many decades in various Mediterranean countries. Although regional cuisines differ between Greece, southern Italy and Spain, they share common core principles.

Few dietary patterns have been studied as extensively over recent decades as the Mediterranean diet. The evidence is particularly convincing for cardiovascular disease, metabolic health and all-cause mortality.
Cardiovascular health
The strongest scientific evidence concerns cardiovascular health. Several large meta-analyses show that high adherence to the Mediterranean diet is associated with a lower risk of heart attack, stroke and cardiovascular death.
Several mechanisms acting simultaneously are thought to contribute. The diet often improves the lipid profile, supports normal vascular function and helps regulate blood pressure. At the same time, it provides large amounts of unsaturated fatty acids, fiber and antioxidant plant compounds.
Metabolism and type 2 diabetes
The association with type 2 diabetes is also now well established. Studies show that people following a Mediterranean dietary pattern are less likely to develop type 2 diabetes and, if they already have the condition, often achieve better blood glucose control.
Several factors likely work together here. High fiber intake slows the rise in blood glucose after meals. At the same time, unsaturated fatty acids improve insulin sensitivity, while the overall favorable dietary pattern reduces chronic metabolic stress.
Brain health
The effects on the brain are also being studied increasingly. Several observational studies associate high adherence to the Mediterranean diet with a lower risk of cognitive impairment and dementia.
The causes are likely multifactorial. Improved vascular health, lower levels of inflammation and better overall metabolic health may together help support brain function over the long term.
Mediterranean diet and the Hallmarks of Aging
The Mediterranean diet is not only associated with a lower incidence of chronic disease. Scientists are also increasingly investigating whether it influences biological processes that shape the aging process itself.
What is particularly interesting is that the Mediterranean diet does not act through a single mechanism, but influences several signaling pathways and metabolic processes at the same time. It is precisely this complexity that makes it so relevant from a longevity research perspective.
One important target is chronic low-grade inflammation, which often increases with age and is referred to as inflammaging. A diet rich in vegetables, fruit, legumes, nuts and olive oil provides numerous inflammation-modulating plant compounds and unsaturated fatty acids. Several studies show that people with high adherence to the Mediterranean diet often have lower concentrations of inflammation-associated biomarkers such as C-reactive protein or interleukin-6.
Mitochondrial function may also benefit. Mitochondria produce most of the energy used by cells and play a central role in healthy aging. Experimental studies suggest that polyphenols, monounsaturated fatty acids and other components of the Mediterranean diet may support mitochondrial function and limit the formation of reactive oxygen species.
In addition, the Mediterranean diet influences various metabolic pathways associated with insulin sensitivity and metabolic flexibility. Since chronically elevated blood glucose and insulin levels are associated with numerous age-related diseases, this mechanism is considered particularly relevant.
Potential effects on oxidative stress, the gut microbiota and epigenetic processes are also being discussed. Many of these relationships are biologically plausible, but their long-term significance for human aging continues to be investigated.
What do large intervention studies show?
The PREDIMED study represents a milestone in nutrition research. It is one of the most important randomized intervention trials of the Mediterranean diet and provides substantially stronger evidence than purely observational studies.
Several thousand people at increased cardiovascular risk participated in the study. They were followed for years and received either a low-fat diet or a Mediterranean diet supplemented with extra virgin olive oil or nuts.
The most important result was clear: the Mediterranean diet groups experienced fewer major cardiovascular events such as heart attack, stroke or cardiovascular death than the control group.
In the following years, numerous additional analyses of the same study population were published. Among other findings, they reported indications of positive effects on type 2 diabetes, certain cognitive functions and individual metabolic parameters.
Several meta-analyses now also confirm that high adherence to the Mediterranean diet is associated with lower all-cause mortality and a lower risk of various chronic diseases.
Nevertheless, these results also need to be interpreted carefully. Even high-quality nutrition studies cannot be completely blinded. In addition, people often change several lifestyle factors at the same time, making it difficult to separate individual effects with complete certainty.
The PREDIMED study is one of the most convincing intervention trials in nutrition research. It provides strong evidence that a Mediterranean diet can reduce the risk of major cardiovascular events.

Intervention studies show that extra virgin olive oil can improve vascular function and reduce oxidative stress. For this reason, it is considered an important contributor to the health effects of the Mediterranean diet.
Nevertheless, its importance should not be overstated. Olive oil alone does not make a Mediterranean diet. It is the combination with vegetables, whole grains, legumes, nuts and fish that creates the dietary pattern studied in major trials.
Mediterranean diet and a healthy body weight
Although the Mediterranean diet is not primarily designed for weight loss, it is often associated with a healthier body weight and a lower risk of overweight.
One major reason is its high nutrient density. Vegetables, legumes, whole grains and fruit provide large amounts of fiber and water. As a result, they often provide greater satiety than highly processed foods with a high energy density.
High-quality fat sources such as olive oil or nuts do not counteract this effect. Although they are energy-dense, numerous studies show that they do not automatically lead to weight gain when consumed as part of a Mediterranean diet. On the contrary, they may promote satiety and reduce the consumption of highly processed foods.
In addition, the Mediterranean diet contains only small amounts of added sugar and highly processed products. This makes it easier for many people to align energy intake with their individual needs over the long term without consciously counting calories.
The energy balance remains decisive, however. A Mediterranean diet leads to weight loss only if energy intake remains lower than energy expenditure over the long term.
Can the Mediterranean diet be followed anywhere?
It is often assumed that a Mediterranean diet is only possible in Mediterranean countries. In reality, however, it is based less on specific regional products than on general dietary principles.
Vegetables, legumes, whole grains, nuts and high-quality plant oils are now available almost everywhere. Local foods can also be integrated into this dietary pattern without difficulty. Depending on the region, lentils, beans, oats, berries or rapeseed oil can be useful alternatives or additions.
The decisive factor is therefore not whether a food originally comes from Italy or Greece. What matters more is that the diet consists predominantly of minimally processed plant foods, while animal products and highly processed foods play a smaller role.
This flexibility in particular helps explain why the Mediterranean diet is often easier to maintain over the long term than highly restrictive diets.
The Mediterranean diet is not a fixed meal plan. It describes a dietary pattern that can be adapted to regional foods and personal preferences.
Common misconceptions about the Mediterranean diet
Numerous misconceptions surround the Mediterranean diet, and many of them only partly align with the scientific evidence.
"Olive oil alone makes a Mediterranean diet healthy."
Olive oil is an important component, but it does not explain the health benefits on its own. The difference comes from the interaction of many plant-based foods, high-quality fats and overall balanced eating habits.
"The Mediterranean diet means unlimited fat."
Although olive oil and nuts are central components, total energy intake still matters. High-quality fat sources replace other fats; they are not simply added in unlimited quantities.
"Red wine is an essential part of it."
Earlier observational studies partly attributed health benefits to moderate wine consumption. Today, this assumption is viewed much more critically. Current evidence does not support recommending alcohol for health reasons. The positive effects of the Mediterranean diet can also be achieved without wine.
"The Mediterranean diet is only useful for heart patients."
The scientific evidence now extends far beyond cardiovascular health. Metabolism, brain health and various markers of healthy aging are also associated with high adherence to the Mediterranean diet.
Limitations of the current evidence
Although the Mediterranean diet is one of the best-studied dietary patterns, some questions remain unanswered.
A large proportion of the available data comes from observational studies. These show consistent associations between the Mediterranean diet and various health outcomes, but they do not allow definitive conclusions about cause and effect.
Randomized intervention trials such as the PREDIMED study substantially strengthen the evidence, but their findings can only be generalized to all population groups to a limited extent. Dietary habits differ considerably between countries, cultures and age groups.
In addition, diet is always part of a broader lifestyle. People with high adherence to the Mediterranean diet often exercise more, smoke less and generally engage in more health-conscious behaviors. Such influencing factors can be statistically adjusted for, but never completely excluded.
Research on the Hallmarks of Aging is also still in its early stages. Many biological mechanisms appear plausible and are supported by experimental studies. Whether they actually lead to a measurable slowing of biological aging in humans must be investigated in further long-term studies.
Conclusion
The Mediterranean diet is one of the most extensively studied dietary patterns in science. The evidence is particularly convincing for better cardiovascular health, a lower risk of type 2 diabetes and lower all-cause mortality.
Its greatest advantage, however, does not lie in individual foods. The overall dietary pattern is what matters. A high intake of plant-based foods, high-quality sources of fat, regular fish consumption and a low proportion of highly processed foods work together and complement one another.
From a longevity research perspective, the Mediterranean diet is also particularly interesting. It influences several biological processes associated with healthy aging, including inflammation, metabolic health and possibly mitochondrial function. At the same time, it must be acknowledged scientifically that many links to the Hallmarks of Aging still require further investigation.
For practical purposes, however, the picture is clear: The Mediterranean diet is one of the best-supported dietary strategies for healthy aging. It can be flexibly adapted to different eating habits and provides a scientifically grounded foundation for long-term health-promoting nutrition.
